        <title id="abstract-title-c65d651c607c4d9cb95923a06018c708">Abstract</title>
        <p id="paragraph-abfcbf5ce5bc4881bba10bf90dc522c3"><bold id="strong-6787488e80704fb1ac73cdd8090f7a41">Objective:</bold> Black cumin (<italic id="e-91d9b3cc3b7c">Nigella sativa</italic> L.) seeds and its crude extract or essential oils have been widely used traditionally for nutritional and medicinal applications in Asian countries. Its effects on diabetics with high fat diet consumption have not been adequately studied. This study was undertaken to study its effects on body weight, abdominal girth, lipid profile and plasma glucose levels in diabetic high fat diet fed rats. <bold id="strong-2e2388c37cda40c7bb5fcb2c84a29724">Materials and </bold><bold id="strong-21dfd1af1e9e4ad699074fc5ac471d3f">Methods</bold><bold id="strong-11a57aeceb524a278c9d73b75cad9355">: </bold>Streptozotocin induced male Wistar rats were fed high fat diet [5130 kcal] for 4 weeks after which they were given vehicle, <italic id="e-113b4f5403e6">Nigella sativa</italic> ethanolic extract (300mg/kg), <italic id="e-df7c2192a28b">Nigella sativa </italic>ethanolic extract (600 mg) or metformin (100 mg). Lipid profile, blood glucose, body weight &amp; abdominal girth were measured. <bold id="strong-65bcc3ca1e17499399e099ec984f04e1">Results:</bold> <italic id="e-c00bc4c42b33">Nigella sativa</italic> ethanolic extract (600 mg) caused significant reduction in blood glucose, total cholesterol, triglycerides, VLDL and non HDL cholesterol comparable to metformin. <bold id="strong-25b829c6be184bb39826e5ffd780a08a">Conclusion</bold><bold id="strong-7f1387fcd3144f1b896055e494699af9">: </bold><italic id="e-3f5a9429c3c2">Nigella sativa</italic> shows anti-hyperglycemic effects and improvement in lipid profile in diabetic high fat diet fed rats which is comparable to metformin. Further studies are required to advocate its use in patients with diabetes and dyslipidemias.</p>

      <p id="paragraph-2c2cdff605b04b409aced390522fadda">Diabetes has become a major cause of morbidity and mortality and its global prevalence is growing rapidly. According to International Diabetes Federation there were 366 million people with diabetes in 2011 and this is expected to rise to 552 million by 2030 <xref id="xref-d593d2bcb8ce4d188cda41b468d370c2" rid="R163031626713688" ref-type="bibr">1</xref>. The epidemic of diabetes has been linked to change in modern lifestyle and rise in obesity <xref rid="R163031626713697" ref-type="bibr">2</xref>, <xref rid="R163031626713696" ref-type="bibr">3</xref>. Susceptible over nourished individuals develop type 2 diabetes owing to the failure of adaptive responses to safely dispose of the fuel surfeit <xref id="xref-6dd6bc0561ed44cc98c090da29b85542" rid="R163031626713690" ref-type="bibr">4</xref>. India is the diabetes capital of the world; every fifth diabetic in the world is an Indian <xref id="xref-b55cf412f8c84276ad6e0d8896910e83" rid="R163031626713675" ref-type="bibr">5</xref>. Since India has a rich wealth of medicinal plants and the potential to accept the challenge to meet the global demand for them <xref id="xref-d209ee752ec64be9a11496b79ade5df1" rid="R163031626713693" ref-type="bibr">6</xref>, Indian medicinal herbs need to be explored for their efficacy in obese diabetics.</p>
      <p id="paragraph-6619c9554b6f4b94a479ea3ec2f74bac"><italic id="e-6e58bddb17a6">Nigella sativa</italic> (NS) (Black cumin), an annual herbaceous plant of the Ranunculaceae family, is a commonly used spice in Indian food and has been used for medicinal purposes in Asian and African countries <xref id="xref-5b3843bfc10c47a88f9ba1f9a029c0e9" rid="R163031626713676" ref-type="bibr">7</xref>. Several beneficial pharmacological properties have been attributed to it such as antihyperglycemic <xref id="xref-69261fd4d12d4d378b6d849c42196ee5" rid="R163031626713682" ref-type="bibr">8</xref>, antiobesity <xref id="xref-ce1a61d8f9524d1cb692f55efaabdc7d" rid="R163031626713674" ref-type="bibr">9</xref>, antihyperlipidemic <xref id="xref-446ad7ca045f46e889408a17e4f2f171" rid="R163031626713691" ref-type="bibr">10</xref>, antimicrobial <xref id="xref-eee3612454674037a37aa84490507474" rid="R163031626713700" ref-type="bibr">11</xref>, antiviral <xref id="xref-03cc04c8d1e44434a5982adaa47eac14" rid="R163031626713699" ref-type="bibr">12</xref>, hepatoprotective<xref id="xref-1515e5bf203b406db13bc7dd692c3d8a" rid="R163031626713685" ref-type="bibr">13</xref> and nephroprotective <xref id="xref-028f454105714a58b90bba7d076e524a" rid="R163031626713686" ref-type="bibr">14</xref> activities. <italic id="e-bcb6ec273a20">Nigella sativa</italic> extracts have shown promising results as an antidiabetic agent in Streptozotocin-induced diabetic rats by exerting a therapeutic and protective effect on the pancreas, by decreasing oxidative stress and ﻿preserving B-cell integrity <xref id="xref-d7cce206ead44d7d856d8da341dbcdfb" rid="R163031626713679" ref-type="bibr">15</xref>. It has also been found to increase serum insulin, and decrease the diabetes induced hyper-glycaemia <xref id="xref-35b6f686506c438085801b1ec898c4bd" rid="R163031626713695" ref-type="bibr">16</xref>. It has also shown favorable efficiency on total cholesterol and Low Density Lipoprotein (LDL) reduction and has anti-atherogenic properties<xref id="xref-c685d17f192e44638e6ca553d9ab344d" rid="R163031626713673" ref-type="bibr">17</xref>  by two main mechanisms; first uptake of LDL-cholesterol via up regulation of LDL receptor gene and second, inhibition of cholesterol synthesis via suppressing the HMG-COA reductase gene <xref id="xref-9cb7e34e9ebb467286e8bd807d9e33cc" rid="R163031626713680" ref-type="bibr">18</xref>. However, to the best of our knowledge, its effect on diabetic rats on high fat diet has not been studied.</p>
      <p id="paragraph-7e66775e65134dc2b3cd12517b071158">In our present research we have studied the effects of NS seeds on lipid profile and plasma glucose level in diabetic rat models that have been fed with high fat diet. These parameters were also studied in experimental animals treated with metformin which helped us to compare the effectiveness of alternative preparation-NS with that of metformin. </p>
    </sec>
    <sec>
      <title id="title-3b4e805827144c0ebdd469ad4e5b58da">Materials and Methods</title>
      <sec>
        <title id="title-5e14d8d6f18f4171a3938c63dd162c4b">Materials</title>
        <p id="paragraph-55a732c5a0b74c338a3bba626aac8867"><italic id="e-5fa9e7addd9c">N. sativa</italic> seeds were obtained from NBRI (National Botanical Research Institute), Lucknow and were authenticated by a botanist at the same institute. High fat diet (HFD) was composed of 300 g concentrates, 350 corn, 300 g beef tallow, 50 g vitamins, minerals and fibers. Composition of HFD was 20% crude protein, 35% fat, 40 % CHO (starch 35%, 5% sucrose) 5% vitamins and minerals and fibers. Metabolic energy of this diet was 5130 Kcal /kg, 61% of this energy was from fat <xref id="xref-46175f64bcd440d586f9b6033e8b176f" rid="R163031626713698" ref-type="bibr">19</xref>. Streptozotocin was obtained from Sigma Aldrich, USA. Metformin was procured from Novartis, India</p>
      </sec>
      <sec>
        <title id="title-cfa7ce86286a4ca293b142cd930c3f4d">Extract preparation</title>
        <p id="paragraph-b5c506e6101a478292d618c7262bb8ef">Ethanolic extract of <italic id="e-b50214cd3ff3">N. sativa</italic> seeds was prepared by maceration. 50 mg seeds were soaked in 150 ml analytical grade ethanol for 7 days with frequent stirring with a sterile glass rod. After 7 days, extract was filtered using Whatman Filter paper- No. 1 and the filtrate concentrated using Rotary Evaporator and stored at 4°C till further use <xref id="xref-6b28d6054af448d982aa1328ac29ff8d" rid="R163031626713677" ref-type="bibr">20</xref>.</p>
      </sec>
      <sec>
        <title id="t-b3811898375d">Experimental animals</title>
        <p id="paragraph-a40bf9b96fa74a959aa8d32d50f173a4">All experimental procedures were performed after approval from Institutional Animal Ethics Committee and as per the guidelines of Animal Care by, Committee for the Purpose of Control and Supervision of Experiments on Animals (CPCSEA), India. Male Wistar rats (200-250 gm) were obtained from Central Drug Research Institute, Lucknow. Animals were kept in the institutional animal house under 12 hour day and night cycle under standard conditions. Rats were given food and water ad libitum.</p>
      </sec>
      <sec>
        <title id="title-0196f41ab02f4bcd8a6587a73451c4ef">Experimental design</title>
        <p id="paragraph-329e5a9499ca42b9a50cc231c2d2780a">Diabetes mellitus was induced using Streptozotocin <xref id="xref-ab93fbaf0f2f4ebcb3c6371b7a4dbcf9" rid="R163031626713701" ref-type="bibr">21</xref>. After overnight fasting, single intra-peritoneal injection of Streptozotocin 50mg/Kg was given. Then the animals were allowed to drink 5% glucose solution overnight to overcome drug-induced hypoglycemia. After a week’s time for development of diabetes, diabetic rats (blood glucose above 250 mg/dl) were fed a HFD for 4 weeks and then divided into following four experimental group of 6 rats each.</p>
        <p id="paragraph-c5e65c57bd5e47e98f8922c16bfca774">Control Group: Diabetic control was given Distilled Water orally.</p>
        <p id="paragraph-8a297f49c464434881d17785a71379f2"><italic id="e-fa63c04b3124">N. sativa</italic> 300: animals were given <italic id="e-4051b757470e">N. sativa</italic> ethanolic extract 300mg/kg body weight orally for 7 days.</p>
        <p id="paragraph-9e55cc263697463ca64cd7880d5a86e9"><italic id="e-ef186b288167">N. sativa</italic> 600: <italic id="e-2f860dae6a78">N. sativa</italic> ethanolic extract 600mg/kg body weight orally for 7 days.</p>
        <p id="paragraph-41de4c269a65448595151bf33d00cc91">Metformin Group: Metformin 100mg/kg/day orally for 7 days.</p>
      </sec>
      <sec>
        <title id="title-11800548f56c4822b37863dcb0e3676b">Hypoglycemic and hypolipidemic activity</title>
        <p id="paragraph-08f0190e50434dcebc4386b495d7ab50">On Day 0 and Day 7 of the study 1ml of blood was withdrawn from tail vein and sent for analysis of lipid profile and blood glucose. Lipid profile and blood glucose were measured using autoanalyser.</p>
        <p id="paragraph-1735f7a43bef44bba66b864ecd66bc64">Abdominal circumference (AC) <xref id="xref-49ab9d850f304faa958e0f20e3704a9f" rid="R163031626713670" ref-type="bibr">22</xref><bold id="strong-0ea6a88444cc440197b3c39ff47c4520"> </bold>was assessed on the largest zone of the rat abdomen using a plastic non extensible measuring tape with an accuracy of 0.1 cm. AC corresponds to visceral fat mass in rodents.</p>
      </sec>
      <sec>
        <title id="title-f9242e275bec45e2b476ba9d74e42483">Statistical analysis</title>
        <p id="paragraph-f435c83232264c0d881ead30a68c6ffe">ANOVA was used for comparison of lipid profile, blood glucose, body weight and AC among different groups. Relationships between body weight, AC and response to drug treatment were measured using Pearson’s Test.</p>
        <p id="paragraph-292c97fc634044fa883168befbb18797">All the analyses were done using SPSS 17.0 version and P value&lt;0.05 was considered as significant.</p>
      </sec>
    </sec>
    <sec>
      <title id="t-27a0ad8329df">Results</title>
      <sec>
        <title id="title-cfb02a1acc9644f087ddbbb4106670d0">Change in body weight and abdominal circumference</title>
        <p id="paragraph-824cc39fd9e944bead2f48d2620a045c">There was a significant increase in body weight (BW) and AC of all the DM rats fed on HFD as shown in <xref id="x-82f29c5ba342" rid="table-wrap-f836d17510e24b4ebe01aa534a93f563" ref-type="table">Table 1</xref>. According to table 1.2 no significant difference was seen between the various groups in BW, AC, blood glucose, lipid profile – total cholesterol (TC), High Density Lipoprotein (HDL), Triglyceride (TG), Low Density Lipoprotein (LDL), Very Low Density Lipoprotein (VLDL) – at day 0 before starting treatment. After 7 days of treatment with vehicle, NS 300mg/kg, NS 600 mg/kg or metformin 100mg/kg, no significant difference was seen in BW (p &gt;0.05) or AC (p &gt;0.05).</p>

        <p id="paragraph-1db09d5912bf40d3839abf1de6fd298c">A highly significant difference (p &lt; 0.001) was seen in glucose levels of the various groups.</p>
        <p id="paragraph-3a947765c44b489e9a8c9fe5a82223c1">A significant decrease in blood glucose was seen in rats treated with NS or metformin as compared to control group as is seen in <xref id="x-3827fd759aa4" rid="table-wrap-3bbf5dc4f4064178b67eb4e0ce6c0b4e" ref-type="table">Table 2</xref>. No significant difference was seen between the groups taking NS or metformin indicating comparable efficacy of these drugs.</p>

          <p id="paragraph-7f5b5dc8d3894162835c48cb378fdaeb">At day 7, the HDL levels in all the groups were similar (p &gt;0.05) showing that NS or metformin did not significantly affect HDL levels as seen in <xref id="x-9156c4a61698" rid="table-wrap-8814f469589b4b44ad840b9a7289d6a6" ref-type="table">Table 3</xref>.</p>
        </sec>
        <sec>
          <title id="title-693d7a1b6c9c4b4bbcfe51e538d2e611">Triglycerides</title>
          <p id="paragraph-cf2c07533448488ebd59c3071a07bdd6">A highly significant difference was seen between the different groups in TG levels on day 7. However, TG level was significantly lower than control only in group receiving 600 mg/kg NS extract as seen in Table 3.</p>
        </sec>
        <sec>
          <title id="title-a7b12c7cc7154e0fb4c05644e129f723">Low Density Lipoprotein</title>
          <p id="paragraph-e0e83e810fbc4c4d8e79b8a9e7dcdec3">hows highly significant difference (p &lt; 0.001) at day 7 in LDL levels of different groups.LDL levels were significantly lower in groups receiving NS or metformin. No significant difference was seen between NS and metformin groups.</p>
        </sec>
        <sec>
          <title id="title-bfbbd17d79ac4e38822c9d37170ef8f1">Very Low Density Lipoprotein</title>
          <p id="paragraph-80cb3e962a284de790e9430c8e28c864">A highly significant difference was seen between the different groups in VLDL levels on day 7as shown in <xref id="x-1d55a6c665b0" rid="table-wrap-8814f469589b4b44ad840b9a7289d6a6" ref-type="table">Table 3</xref>. However, VLDL was significantly lower than control only in group receiving 600 mg/kg NS extract.</p>
        </sec>
        <sec>
          <title id="title-f0da0330c689435490d21af5894ae8f2">Correlation between body weight and effect of <italic id="e-79d09ed32afa">Nigella sativa</italic></title>
          <p id="paragraph-91198c0ae0a44c1ea187b0507157bf73">As seen in <xref id="x-c20d2daab90d" rid="table-wrap-279dfaaa69f44696950e6cd39aba0630" ref-type="table">Table 4</xref> and <xref id="x-660e661ba57e" rid="f-b4f1756443dc" ref-type="fig">Figure 1</xref>  glucose levels at Day 7 in rats receiving 600 mg/kg NS extract were found to be negatively correlated with their body weight.</p>

      <title id="t-50ed6e5712a9">Discussion</title>
      <p id="paragraph-a04dae26e2834d599758894510c237e7">Our study is in line with that of Qidwai et al, 2009 <xref id="xref-b678954db0814c509642c96e3d50f601" rid="R163031626713692" ref-type="bibr">23</xref> who reported that NS seeds reduced BW, BMI and waist-hip ratio, but the results were not statistically significant. In our study a highly significant decrease in glucose levels was seen after administration of NSE which was comparable to metformin. This is in confirmation with the study of Al Awadi FM et al, 1985 <xref id="xref-7ddd5254c34c4d0d8b226337b76372bc" rid="R163031626713678" ref-type="bibr">24</xref>, who reported a significant decrease in blood glucose produced by a plant mixture containing <italic id="e-0295f76b144c">N. sativa.</italic> </p>
      <p id="paragraph-a5e5c44b95c14f1f8224efefbdc0ecc1">Our study also confirms the study of Fararh et al., 2002<xref id="xref-f6f3d5bbf31f4be58937626c037c8b7c" rid="R163031626713694" ref-type="bibr">25</xref> and El-Dakhakhny et al., 2002<xref id="xref-8aaf8ae84d0749d590414b73bd9217ec" rid="R163031626713681" ref-type="bibr">26</xref> who reported anti-hyperglycemic effects of NS oil in Nicotinamide-STZ induced DM hamsters, and STZ DM rats respectively.</p>
      <p id="paragraph-b3fb9dc990104f0fbc754a6f499b27f5">In the present study NS effectively reduced TC, TG and VLDL levels. This confirms earlier studies which showed significant decrees in VLDL, TG and TC levels, by NS seeds in Wistar Albino rats<xref id="xref-6a405c00e30d4ba08936aa4bc63fec76" rid="R163031626713684" ref-type="bibr">27</xref> as well as in humans <xref id="xref-c2bfac72ff6f4abca5d84aad8862813d" rid="R163031626713692" ref-type="bibr">23</xref>. However, no significant change was seen in HDL and LDL levels. Kocyigit et al, 2009 <xref id="xref-4223e992b00241a39f666cd678a8636f" rid="R163031626713680" ref-type="bibr">18</xref> reported a significant decrease in LDL and an increase in HDL levels in rats. The difference may be because the rats used in this study were non-diabetic while we used diabetic rats in our study. </p>
      <p id="paragraph-9948aa6d421d4af69192e92d4ab0343e">A negative correlation was seen between BW before start of treatment and blood glucose levels at the end of NS treatment. This shows that NS is more effective in reducing BG if BW is higher. The reason for this is that higher BW is associated with insulin resistance. NS treatment has been shown to improve glucose tolerance as effectively as metformin <xref id="xref-a4ec552d0b8445dcaec590244b3b152e" rid="R163031626713680" ref-type="bibr">18</xref>.</p>
      <p id="paragraph-c7786ad33e89459986aae2799dd8e8c2">Our study also confirms the study of Najmi A et al <xref id="x-9de3aeb2ee3f" rid="R163031626715412" ref-type="bibr">28</xref>  who reported that NS treatment significantly reduced blood glucose levels in patients of Insulin Resistance Syndrome. The anti-hyperglycemic activity of NS may be due to insulino-tropic <xref id="xref-5301707d0f224f8ea4cafd6a1ebd7b5a" rid="R163031626713694" ref-type="bibr">25</xref> as well as extra-pancreatic <xref id="xref-ef0ab9cc7166426f94ad0c56ca6189f6" rid="R163031626713681" ref-type="bibr">26</xref> effects of NS which may be attributed to the presence of Thymoquinone <xref id="xref-5eb4d074301e4237ade4df704c8c53c6" rid="R163031626713695" ref-type="bibr">16</xref>. The antihyperglycemic and hypolipidemic activity of NS coupled with its antioxidant activity against atherogenic diet induced stress <xref rid="R163031626713684" ref-type="bibr">27</xref>, <xref rid="R163031626713680" ref-type="bibr">18</xref> shows that it can be used as an important food supplement in diabetic patients taking high fat diet.</p>
      <p id="paragraph-0e6ff8cac91a4156950329e6eefd8405">Our study reported superior effect of NSE (600mg/Kg) on Triglyceride levels compared to Metformin, apart from similar effects on other lipid profile parameters, blood glucose and body weight. Moreover several studies have shown beneficial effects of <italic id="e-b1bb7ec7b513">Nigella sativa</italic> in prevention or treatment of various comorbidities in people with diabetes, like reduction of CRP levels <xref rid="R163031626713672" ref-type="bibr">29</xref>, <xref rid="R163031626713687" ref-type="bibr">30</xref>, antioxidant activity <xref id="xref-3b323663664c449db04640a22994df1d" rid="R163031626713687" ref-type="bibr">30</xref>, nephroprotective effect <xref id="xref-61b598ad88aa4e1c8bdf6e0303a322c4" rid="R163031626713689" ref-type="bibr">31</xref>, and improvement of endothelial function <xref id="xref-c8671c1165ea40b98015912596e65924" rid="R163031626713671" ref-type="bibr">32</xref>. This indicates the positive implication of using the NSE over metformin</p>
    </sec>
    <sec>
      <title id="t-4fe21a9d5ffc">Conclusion</title>
      <p id="paragraph-beaed03077c04343939cc044cae6bb73"><italic id="e-44328174d127">Nigella sativa</italic> shows anti-hyperglycemic effects in diabetic high fat diet fed rats which is comparable to metformin. It also causes decrease in TC, TG and VLDL. Although further studies are required to advocate its use in obese DM patients.</p>
